New Patient Forms
In order to provide you with the most efficient new patient experience, please download and complete the forms that apply below and bring them to your first appointment. This will allow the new patient process to run quicker and smoother for both our practice and you.
Our team will take comprehensive records and address any concerns or questions you may have. If you are having problems downloading the forms, please contact our office and we will gladly send them to you.
Insurance and Billing
We can help you maximize your benefits. We are more than happy to submit all insurance forms for you. It is recommended that you contact our office prior to your appointment to provide us with insurance information for verification of benefits. We accept most dental insurance plans and we accept check, cash and all major credit cards.
Some dental insurances we are in-network with include Premier Delta Dental, MetLife, WV Chip, and Guardian. We are not in-network with Cigna, but we do participate with them.
We are pleased to offer our patients CareCredit as financing. CareCredit can help you pay for treatments and procedures over a period of time that your insurance doesn’t cover.

If you are unable to keep your scheduled appointment, please kindly provide us with at least 48 hours notice by calling our office at (304) 725-0508. An additional fee may be charged if a patient does not show up for an appointment without sufficient notice.
